Familiar Roads Home Healthcare is an approved provider through Pennsylvania’s Office of Developmental Programs (ODP).
We partner with Supports Coordinators (SCs) across Bucks, Montgomery, Chester, Delaware, Philadelphia, Lehigh, and Luzerne Counties to deliver Home- and Community-Based Services (HCBS) under the Consolidated, Community Living, and Person/Family Directed Support (P/FDS) waivers.

Pennsylvania’s ODP waivers provide people with intellectual disabilities, autism, or developmental disabilities with the supports they need to live safely and independently at home and in their communities.

In-Home & Community Supports | Daily living assistance, personal care, and skill-building activities to promote independence and self-confidence. |
Companion Services | Social and safety support to encourage inclusion, friendship, and community engagement. |
Respite Care | Short-term relief for family caregivers—available in home or community settings. |
Homemaker / Chore Services | Household tasks such as laundry, cleaning, meal preparation, and home organization to ensure a safe environment. |
Community Participation Support (CPS) | Volunteer, educational, and recreational activities that help participants develop skills and build community relationships. |
Housing Transition & Tenancy Sustaining | Support to find, secure, and maintain safe and affordable housing in the community. |
Transportation (NEMT) | Door-to-door rides to appointments, employment, day programs, and social events for waiver participants. |
Behavior Support Coordination | Collaboration with behavior specialists to develop and implement positive behavior plans. |
Supports Coordination Collaboration | Ongoing communication with your SC to monitor progress and update service plans. |
